If you are a new patient to Cancer Specialists of Greater Dayton, please download and print the forms and complete prior to your visit. Please arrive 30 minutes early to your appointment in order to accommodate the check in process; your physician may require blood-work to be drawn prior to your appointment, which may be done at the office. If blood-work and ordered tests cannot be performed at our office, our new patient intake coordinator will arrange those appointments and will contact you in regard to those appointments. Also, please bring ALL of your medication bottles with you to your appointment. It is imperative that your physician is aware of all medications you are taking regularly and as needed, including vitamins and herbals.
